Multi Channel Communications with Patients in Primary Care

Problem Statement

“The Virtual Hospital” is an NCA-wide attempt to mitigate COVID-19 and support GPs and care homes to deliver Virtual Access to Clinicians & Virtual access to patients in nursing homes, care homes or their own home.  As part of this initiative, need to provide the ability to gather additional data and communicate with Primary Care patients in a variety of methods - including text messages, communication campaigns, satisfaction surveys, collect self-monitoring data and Video Consultation.

What was the solution?


Remote Consultationreduces face to face appointments by enabling healthcare professionals to make informed decisions for triaging patients and resolve requests remotely.

Messagingenables healthcare providers to communicate directly with patients through multi-channel messages.  This in turn reduces DNA’s and frees up appointments for patients who need them.  In addition, improves uptake and awareness of health campaigns through local and personalised messaging.  The solution automates appointment reminders for patients and can seamlessly integrate with healthcare systems.

Communication Channel – delivers large scale and multi channel patient communication campaigns.  Clinically proven to improve patient engagement.  Provides customisable messaging across multiple media channels.  Ability to target and recall specific clinical patient groups at scale.  Read / write interactions directly into the patient medical record.

Patient App - provides an interface for health care access and delivery.  Patients benefit from appointment booking, cancellations & reminders.  Tools to manage medication reminders, ordering of repeat medication and tracking adherence.  Patients can create a circle of care to manage family’s health.  Self-care modules available for long-term condition management.  Clear interface between patients and health care providers
